Families with students enrolled in the Carlisle Schools for the 2009-2010 school year will need to complete the following forms:

• Registration and Student Information Form
  • Textbook rental fee amounts are included on this form.
• Release of Student Records Form
• Language Survey
• Student Release of Information Form
  • Complete only if you are denying the district permission to release information.
• Military Opt-Out Form
  • This form pertains only to high school students. Complete if you are denying the release of information to college, athletic, or military recruiters.
• Transportation Forms
  • Students who only ride the shuttle bus to Hartford Upper Elementary or the Middle School will not need to complete a transportation form.
  • Regular Transportation Form
    • Complete if your student will be using the bus service outside of the Carlisle city limits.
  • Discretionary Transportation Information
  • Discretionary Transportation Form
    • Complete if you will purchasing bus service within the Carlisle city limits.
• Athletic Physical and Insurance Waiver Form
  • This form is required for all students in grades 7-12 participating in athletic activities.
• Free and Reduced Application
  • This information will be updated this summer as it is received from the Department of Education. An application needs to be completed each school year.
• Complete the Fee Waiver Form if you have received a Free and Reduced Application
• 6th - 8th Grade Band/Chorus Form
